The 2024-20245 Application Process for applicants is NOW CLOSED. We will continue to collect late recommendation forms until the end of February 2024. All applicants will receive notification by May 1st, 2024. Our next Baer Reintegration Scholarship application period for the 2025-2026 academic year will begin in early October, 2024.

We want to make it as easy as possible for references to complete their recommendation forms. Should you for any reason be unable to fill out the recommendation form that was sent to you through our data encrypted online portal, Smarter Select; or through our paper application, we have a number of options for you.  These entail either a separate method of sending in your form, or sending in your own letter. The details are laid out below:

Recommendation Format 1: COMPLETING THE RECOMMENDATION FORM

Below are PDFs of our Prescribing Authority and General Recommendation forms that are pulled directly from our mail-in paper application. Should you be a reference downloading this form to submit online or mail by itself, you will notice the top portion asks for the applicant’s name and signature. Please fill in the applicant’s name on their behalf. The signature is not required for you to complete the form. Once we receive it we will alert them via email to confirm the form is received, which will allow an opportunity for the applicant to consent to the reference form. More questions? Recommendation Format 2: WRITE A LETTER OF RECOMMENDATION

Should you be unable to complete our Application Forms, you may also write a letter of recommendation and send it through one of the two options above; by mail or our HIPAA secure server, instead of completing the form.

For Prescribing Authorities Only: Without your recommendation, we are unable to process an individual’s application. Each year hundreds of applicants are disqualified due to lack of involvement from prescribing authorities. Help us break this cycle!

We use your input to verify that each applicant has a qualifying diagnosis that allows them to potentially receive a scholarship from our organization. If you are unable to complete the form or write a detailed letter of recommendation, we will also accept an uploaded or mailed statement on letterhead, including the following: 

The applicant’s name

The applicant’s diagnosis 

The applicant’s medication, if possible

Your signature
